A discussion on a patients story and her transition journey from children to adult services. The session will give a brief overview of what is continuing care and talk about the families experience on what went well, didn’t go well and where AN is today in the adult world !

The session is for any parents/carers, family members or any professionals in the Bedfordshire and Luton area. It may be of particular interest to families who have children with complex needs or who are under the care of the Continuing Care nursing team.
